You will find the new patient paperwork at the link above. Please get it back to us ASAP. Download the attachment and fill it out in detail, read over pages 4-8 and you will sign those in the office. You may need to download Adobe Acrobat (on your iOS device or on your computer) in order to submit the paperwork back to us.
At times, the submission process can be aggravating. In the event you are unable to submit, please send by email to ehlichchiro@bellsouth.net. If it is still frustrating you can print it, fill it out and bring it with you at your appointment or arrive 30 minutes early and fill it out on site. Depending on your computer or phone it can be easy or difficult, so you may have to use one of the above mentioned options.
Covid-19 Policy
If you are exposed to covid before your appointment please reschedule for after your quarantine. If you contract covid please reschedule your appointment for when you are better. If you develop a fever, congestion, headache or cough please reschedule your appointment for after you receive a negative covid test.
